To dial international direct, dial "011" + country code + city code + number.

It may take a while for international calls to connect; you can expect to wait 45 seconds for the ringing to start.
Calls to Canada, Puerto Rico, and many Caribbean countries are dialed the same way as long-distance calls within the United States.
